Responsibilities and Duties
Manage a caseload of 10-13 patients per day, focusing on chronic illnesses like diabetes and hypertension
Conduct 30-minute routine appointments and 15-minute acute visits
Provide monthly follow-ups for weight loss management
Perform annual physicals and sports physicals for children
Address common acute issues such as colds, coughs, and flu
Utilize holistic lifestyle medicine practices to enhance patient care
Work collaboratively with a support team including clinic nurses, receptionists, a health and wellness nurse, and a physical therapist
Additional Information
Must hold Family Nurse Practitioner Certification (FNP)
Required to have Basic Life Support (BLS) certification
Strong communication skills and PC knowledge are highly valued
Must be licensed to practice independently in Illinois
